This applies to SuperSU v1.69 and older.
There appears to be a problem with OTA survival mode and the KitKat OTAs for the Nexus devices. Having OTA survival enabled will make the OTA update fail.
You should disable OTA survial in SuperSU settings before flashing the OTA, and re-root after the OTA is complete.
That issue will probably be solved for the next SuperSU update (v1.7x), but I know a lot of you aren't waiting around for that, so ... :)

chuckling not sure if it was caused by this, but after trying to do the update with survival enabled, the nexus 7(2013) was recovery looped.. Had to do a complete wipe of the device. Like I said... Not sure this caused all that, but just a PSA. ?
I have the free version so the survival mode is disabled right? I can't select the option.
I reloaded 4.3 then updated, I had this issue, I think.

First, They have to release the 4.4 OTA for the nexus 4...
On N7 2012 I unrooted flashed the zip of the OTA in cwm recovery, then immediately flashed the SuperSU 1.6.9 zip.
But it's not taking. I've tried redownloading and flashing the SU zip several times. No luck.
Any ideas? I'm unlocked boot loader on 4.4 with CWM recovery.

I've the free version and when cwm asked if I wanted to reapply supersu I said yes. Now I'm still rooted without reflashing anything else
First let me thank you for your great work chainfire! Up to now everything worked really fine for me!
But I updated my Nexus 7 2013 yesterday and lost root.
Before doing the update I completely disabled (removed?) SuperSU from its settings dialog, then rebooted into recovery(TWRP 2.6.3.1), flashed the kitkat update with wipe caches enabled and directly after that flashed the 1.69 SuperSU ZIP (a procedure which worked fine for the 4.3 update for me).
I have 4.4 on my Nexus 7 now which works fine, but I lost root completely, I tried to reflash the zip file several times, installed SuperSU from the Play Store too, but it now just shows the app-icon in the installed-apps list, but when I try to start it my Nexus tells me "App is not installed".
I'm really stuck here, anything I can still try to regain root access?

Removing daemonsu and .su eg. via adb shell, if stuck in boot loop, will make it possible to run the ota update zip again.

+Salvatore Calaciura I didn't lose root updating with ota zip. Just told cwm to reflash supersu to not lose root
I had the ota notification ( I made an unroot 2 weeks ago) to avoid any problems during update at 4.4. Now I have only the unlock bootloader. Nothing else.
Then flash recovery and root again... I apply ota even if rooted, it's not the first time, never had problems.. BTW usually I apply zip found on internet instead of waiting days or weeks for the update to come up on device by itself

I did a full unroot before flashing the ota, and when I tried flashing the supersu zip it didn't show up in the app drawer and I had a hell of a time getting it installed again. I had to install it from the play store after flashing the zip, catch it before it crashed and do a 'integrate with system' to get it to install properly.
+Krishean Draconis can you detail how you did that. I'm stuck where you were.
+Jimmy Poteat Here's what I did from the beginning:
1) Download the OTA package and the 1.65 update zip onto my Nexus 7 (2012)
2) Restored all files on the /system partition to their stock versions (I had a custom boot animation and the AOSP Browser installed.)
3) Go into SuperSU and hit ''Full unroot."
4) Boot into custom recovery (I use CWM, but I guess any custom recovery will work.)
5) Flash OTA package.
6) Flash SuperSU 1.65 zip.
7) Boot up the tablet into its shiny new OS.
8) Install SuperSU from the play store, as it won't show up in the app drawer.
9) Run SuperSU and hit yes to update su (normal method.)
10) Quickly go into settings and hit "install SuperSU to system" before it crashes (if it crashes just try again.)
11) When the tablet reboots (reboot manually if it doesn't) repeat step 10 once.
12) Hope that it works, if it doesn't, run SuperSU, try either reinstall, switch superuser app, or full unroot and start over at step 6.
If you already have the OTA applied, just boot into recovery and start at step 6.
Oh, and +Chainfire thank you for all your hard work, and sorry if I sent you too many bug reports in my frustration trying to get it to work.

Same here, no go on Nexus7 2012. All other devices rooted fine.

I flashed the 1.69, installed SuperSU through the play store. Because I had 1.69 I wasn't prompted to update, so went to settings and selected install in /system. N7 rebooted. Went to root checker and SuperSU crashed. Back to SuperSU and magically the popup appeared to allow root for root checker.
Not sure the correct process but flashing from recovery, installing from play, and in-app telling it to install to /system fixed it.
Works now!
After update to v1.69 ....i have problem with my root..note 2 leak 4.3 mod note 3....apps use root access not working..restore nandroid..use v1.65 again....?
After having problems yesterday, I just flashed 1.69 again, re-installed it from play store before starting it even once and now have it working again!
My OTA hurts.
